About Rise Functional Medicine
Rise Functional Medicine is a virtual-first functional medicine practice specializing in hormone optimization, gut health, and brain health for active, high-performing individuals — especially athletes and women in perimenopause. We offer high-touch, outcome-driven programs that combine advanced testing, root-cause treatment, and supportive coaching to help our clients feel vital, clear, and strong.
We are growing, and we are looking for a motivated, compassionate Health Coach to join our team as a W-2 employee. This is a long-term role for someone excited to grow with a close-knit, mission-driven practice and contribute as a core member of the team. You will work directly alongside Dr. Ashley Shrader, DNP, IFMCP, to deliver an exceptional client experience and help patients get real, lasting results.
What You Will Do
Patient Coaching & Program Support
You will provide individual coaching to patients enrolled in Rise Functional Medicine programs. This includes conducting coaching sessions approximately every 10–14 days (based on program structure), providing accountability and encouragement as patients implement lifestyle and supplement recommendations, and supporting habit changes related to nutrition, sleep, stress management, exercise, and recovery. You will monitor patient progress using questionnaires and symptom tracking tools, communicate concerns or challenges to the provider when appropriate, and maintain accurate documentation within our electronic medical record system (Practice Better).
Group Coaching & Education
Group-based coaching and education are a cornerstone of how we support patients through their programs. You will support or facilitate scheduled group coaching sessions and educational calls, encourage engagement and accountability within group programs, and help patients implement program materials and educational resources.
Discovery Calls & Patient Enrollment Support
This role includes guiding prospective patients through the decision-making process and helping them determine whether our programs are the right fit. Comfort with these conversations is essential.
You will play an active role in helping prospective patients understand our programs and determine fit. This includes conducting discovery calls, supporting the Program Enrollment workflow, and assisting with patient onboarding and program orientation.
Community Outreach & Educational Marketing
You will contribute to the educational presence of the practice by participating in community presentations, workshops, or events, contributing to social media content (short educational videos, reels, posts), and supporting marketing initiatives that help educate the community about functional medicine and the programs we offer.
Professional Development
You will participate in training modules, attend periodic team meetings and planning sessions, and continue your professional development in functional medicine coaching and patient support. Additional responsibilities may be assigned as the practice grows.
Who We Are Looking For
The ideal candidate is a Certified Health Coach (FMCHC, NBHWC, or equivalent) or someone trained in functional nutrition who is genuinely passionate about hormone and gut health. You are empathetic, organized, and experienced working with high-achieving clients who want real answers and real results. You are comfortable taking initiative, adapting as the practice grows, and being part of a dynamic, evolving role.
You are looking for a stable, long-term position — not a side project. You are comfortable working virtually, but you live in Southern Oregon or the Eugene, OR area so we can connect in person periodically. You are excited to grow with a practice that is doing meaningful work.
Experience in a functional or integrative medicine setting is a strong plus. Familiarity with HRV, CGMs, hormone therapy, AIP or elimination diets, or stool and SIBO testing is a bonus.
Hours, Compensation & Benefits
This is a part-time, hourly W-2 employee position with a clear path to full-time.
Details
Starting Hours: 25–30 hours per week
After 90 Days; Target transition to 32+ hours/week after 90 days based on patient volume, performance in the role, and overall practice growth
Pay Rate: $22–$28/hour, depending on experience and role alignment
Bonus: Eligible for monthly performance-based bonus tied to practice growth and patient outcomes. Bonus potential increases as the role expands and can reach up to $1,000/month.
Location: Mostly remote; must reside in Southern Oregon or Eugene area
